Independent Living Service

At Rykneld Homes, we care about delivering excellent services to our tenants, leaseholder and residents. The primary objective of our Independent Living Service is that of delivering a service which is based upon the personal needs of our residents as opposed to services based on the property that you live in.

The Independent Living Service will provide an enhanced service to residents across all ages and will ensure that any service provided will be based on an individual need and a person centred approach that will cover issues of:

• Individual assessments and completion of a support plan
• Security, health and safety
• Safeguarding and protection from abuse
• Encouraging community engagement, involvement and empowerment
• Identifying early intervention and prevention
• Enabling access to a range of services
• Encouraging individual choice and control

Our Independent Living Officers will work with you to enable you to access the support that you need and will act as the key person between you and a range of services that you need to access to promote independence, develop your self confidence and maximise your enjoyment of life.

Annually a review of your support plan will be undertaken by an independent Living Officer, if your circumstances change at anytime for example you have a period of time in hospital the Independent Living Officer will visit you and update your support plan ensuring that referrals are made to the appropriate agency that can provide you with any additional help that you may require.

Supporting Independent Living

Your support plan will also encourage and help you to undertake tasks within your home that you have previously undertaken or not undertaken in the past and are now experiencing difficulties with for example enabling social activity and community involvement, making appointments or assistance with correspondence.

The Independent Living Service will also provide advice and assistance to residents who wish to consider how to access different housing options, for example if you need to move to a property that has adaptations the Officer will help you consider what type of properties may be coming available with the type of adaptation that will help you to continue to live independently. Advice will also be provided on how to move home including the type of issues that you would need to arrange in order to help you move.

If you are considering moving and do not know what housing options are available within the district an Officer will visit you and provide information on what housing options are available and how often such properties become available for example bungalows and flats within villages. The officer will also complete a support pathway plan. This will help you to consider if you need any support from agencies, enable where agreed referrals to be made to agencies where appropriate before you move home.

In circumstances where you need extra support the Tenancy Support Officer and Neighbourhood Champions will work with you and your family on any issues that relate to your tenancy. Wider community engagement and involvement opportunities are accessible throughout the district the Independent Living Officer will be able to provide information and advice on how to access all involvement, engagement and social activity in your community at the level that you wish to be involved.
