Supporting Independent Living

The Independent Living Service, delivered by Housing & Support Officers, supports vulnerable residents aged 55 and over.

Our Housing & Support Officers will work with our customers to develop an individual support plan, which will help to identify steps that can be taken to improve or help maintain someone's independence.

The service will assist customers to improve the following areas of their lives:

  • Security, health and safety
  • Healthy lifestyle
  • Enabling access to a range of services
  • Money and debt management
  • Safeguarding and protection from abuse
  • Encouraging community engagement, involvement and empowerment
  • Early intervention and prevention
  • Enabling choice and control
  • Keeping active, including contact with friends and family.

The Housing & Support Officers will work with residents in their homes to enable them to access the support they need and act as a key contact between a range of services to promote and bring greater independence, self confidence and enjoyment of life.

We would encourage family, friends, health professionals and any relevant people to attend assessments and reviews.  We believe in working together with other agencies to combine knowledge about the needs of our customers to form a support plan which will genuinely benefit their lives.

The service offered to customers includes:

  • Provision of a Lifeline unit, which is the emergency response equipment
  • Regular support visits, which are tailored to the needs of the individual
  • Creation of a customer-led and focused support plan
  • Annual review of support plan (reviews may take place more regularly as necessary).
